Abstract
The BNPP Tanjung Perak Office in Surabaya plays a strategic role in supporting the success of search and rescue (SAR) operations in the maritime area of East Java. The high mobility of marine traffic and the increasing risk of maritime accidents demand that SAR personnel remain fully prepared to provide rapid and accurate emergency response. One of the essential competencies required of every rescuer is the ability to perform Medical First Responder (MFR) procedures, particularly in managing fracture cases that commonly occur due to ship accidents, falls on deck, strong impacts, and other work-related incidents. Field challenges such as severe weather conditions, high waves, limited medical equipment, and unpredictable emergency situations make the application of MFR techniques crucial in saving lives. This study aims to understand how the effectiveness of the work system at BNPP Tanjung Perak Surabaya is implemented through compliance with Standard Operating Procedures (SOP), personnel professionalism, and the technical ability to apply MFR in real operational conditions. In addition, this research seeks to compare theoretical knowledge gained during education with actual field practice, identify occupational safety barriers faced by personnel, and evaluate the application of fracture management techniques during maritime SAR operations. Data collection methods include land-based practice, direct field observations, interviews with relevant personnel, and literature studies to strengthen the theoretical framework. The findings show that proper application of MFR techniques—such as immobilization, stabilization, and safe victim evacuation—significantly increases the chances of survival before advanced medical treatment becomes available. Moreover, continuous training, adequate medical equipment, and strong understanding of SOPs were found to be key factors in improving personnel performance. This study emphasizes that strengthening MFR competence among SAR personnel and Indonesian seafarers is essential to reducing the fatality rate of maritime accidents and improving the overall quality of emergency response during rescue operations.
